Complete the form below "Consent for Patient Portal"
Fax it to: 845-278-4323
You will then be emailed a Password which you will use to log on.
YOU CAN ALSO VISIT ANY OFFICE LOCATION TO SIGN A CONSENT FORM AND OBTAIN A USER ID and PASSWORD
Through the portal you will be able to:
- Request an appointment
- Email questions to your MD secretary
- Request a refill of medication
- Access a copy of a visit summary
