Office Policy
If you are unable to keep your appointment, please call the office at least 24 hours in advance (845) 278-8400.
Somers Orthopaedics participate with most health insurance plans. Coverage varies with each insurance company. Patients are responsible for deductibles, co-payments and remaining balances. If your plan requires a referral, the Referral Form must be completed by your Primary Care Physician and brought with you to your appointment. If you do not have a referral form, your appointment will be rescheduled. If your plan requires a co-pay, this payment is expected at the time of service. This is a requirement of your insurance company. If we do not have a copy of your current insurance card on file, you will be considered self-pay and payment will be due at the time of service.
Please check with your insurance company if you do not see your plan below.
Your Responsibility: If we do not participate with your insurance, your will be considered self-pay and we will require payment for services rendered due on the day of your appointment. If your insurance company sends the payments to you and not the physician, it is your responsibility to forward both the payment and accompanying explanation of benefits to our office. Any changes in insurance coverage or employment status must be immediately reported to Somers Orthopaedics to ensure accurate submission of claims.
Medicare: As a participating provider, we welcome Medicare patients and are pleased to submit your Medicare and any secondary insurance claim forms for you. Medicare patients are responsible for 20% co-insurance and the Medicare deductible.
Worker's Compensation & NYS No Fault Insurance: If you are injured through your employment or are involved in an accident, we will file your Worker's Compensation or No Fault Insurance claim. You must bring the required information to your appointment so that we may properly process this type of claim. If the forms are not filed with the appropriate agency prior to your appointment, you will be considered a self-pay and payment will be due at the time services are rendered.

Payment Options
We accept cash, checks, money orders, MasterCard, Visa, Discover and American Express. Returned checks are subject to a $20 bank fee.
